Curious --- have you looked into the software stack of the affected CAs - specifically whether they are all EJBCA? If they're all running the same backend, it would explain why the failure mode is so consistent.

It would be interesting to know if the CAs that fixed this quickly are the ones with the technical ability to act independently, while the others are effectively a shell for their vendor contracts.
